Home Health Aide
Our Home Health Aide service completes the care provided within the Home Health program. Our Certified Nursing Assistants provide assistance with a client’s Activities of Daily Living (ADLs). These activities may include toileting, shaving, meal preparation, light housekeeping, and bathing. This type of hands-on “personal care” truly compliments the complete therapeutic realm provided.
The Nursing Bridges Skilled Nursing program provides a wide variety of services and treatments ranging from medication administration, diabetic training, and wound care to IV therapy. In addition, Nursing Bridges Healthcare Inc. realizes the best way to improve health is through education. Our nursing staff focuses on training the client or caregiver to improve compliance or assist.
Medical Social Work
The Nursing Bridges Healthcare Inc. Social Work program focuses on early interventions that may assist in preventing unnecessary hospitalizations or re-hospitalizations relating to social problems. The Nursing Bridges Social Worker seeks to improve family cooperation and enhance the client’s or a caregiver’s involvement in the plan of care. Finally, through frequent contact and assessment, the Nursing Bridges Social Work program hopes to eliminate the frequency or existence of emergency situations.
Therapy Service Details
Nursing Bridges Physical Therapy is a comprehensive program specializing in the evaluation and treatment of orthopedic problems, neurological conditions, joint or muscle dysfunction, posture problems, amputations, head injury, spinal cord injury, stroke, sports injuries, industrial injuries, gait training, restorative therapy, range of motion, therapeutic exercises, and other medically diagnosed pain or dysfunction. This friendly “hands on” care is provided in the safety
and comfort of your home.
Nursing Bridges Speech Therapy is a comprehensive program specializing in evaluation and treatment of speech disorders, swallowing disorders, as well as training with communicative devices. Our Speech Therapists also assist and train clients with verbalizing expression and understanding language in order to maximize communication skills after stroke, surgery or trauma.

Weekend and Evening Admissions
At Nursing Bridges, we recognize that medical care may be required at any time, according to patient needs. Therefore, upon physician or case manager request, a Registered Nurse will perform evening and weekend admissions at times other agencies may refuse to do so.
Bilingual Services: English and Spanish
At Nursing Bridges, we understand the need in our community for bilingual services. That is why our administration and field staff, for the most part, are bilingual in Spanish/English. Upon patient request, a translator will be provided during patient admission. We provides Spanish/English translators 24 hours a day, seven days a week at no extra cost to our patients.
Patient Hospitality Program
We are aware that many patients being discharged from skilled nursing facilities (SNF) live alone and are in need of extra assistance when arriving back home. Upon request, Nursing Bridges will provide our home-bound patients who live alone with a courtesy Home Care Provider Visit (up to 2 hours) to assist in picking up groceries, light housekeeping duties or other domestic services. Again, this service is at no extra cost to our patients.
Hospital/Nursing Home Clinical Record Review and documentations
Upon patient referral and at your request, Nursing Bridges will arrange for a pre-discharge skilled assessment at the facility. A Registered Nurse will consult with the physician, patient, family members and the facility’s discharge planner to identify the patient’s needs and coordinate the services needed. We will give your case manager or designated personnel a report on our findings.
